Surgery for widespread bone and soft tissue necrosis associated with calcium hydroxide extrusion: a case report with submandibular spread
Calcium hydroxide is a commonly used intracanal medication in endodontic treatment; however, uncontrolled extrusion can lead to serious tissue damage. This study presents the surgical management of widespread bone and soft tissue necrosis resulting from calcium hydroxide and barium sulfate extrusion. A 14-year-old female patient presented with severe pain and diffuse firm swelling in the right mandibular posterior region after root canal treatment. Cone-beam computed tomography showed that the extruded material had extended beyond the lingual cortex into the submandibular space, causing bone destruction, while the inferior alveolar nerve was preserved. Surgically, the extruded material was removed, necrotic tissues were debrided, and the affected tooth was extracted. Histopathological examination was consistent with necrosis and a foreign body reaction. No complications were observed in the postoperative period. This case demonstrates that serious tissue destruction can occur even without nerve damage and highlights the importance of early surgical intervention.
